SCHOLARSHIP APPLICATION

SCHOLARSHIP ELIGIBILITY:
Must have been a football player or cheerleader with Middletown Pop Warner.
Must have been accepted full-time at a post-secondary institution.
Must have participated on a high school athletic team.

APPLICATION PROCEDURE:
Application must be received by May15th
Copy of transcript.
Copy of university acceptance.
Complete application

SCHOLARSHIP AMOUNT:
Middletown Youth Football & Cheerleading will grant two awards of $500.00 each. One to a
former football player and one to a former cheerleader.

REQUIREMENTS:
The scholarship committee considers academic performance and athletic participation when
awarding the scholarship. Grade point averages and active participation in interscholastic or
organized sports during school years will be given higher consideration, along with special
consideration given to those interested in pursuing sports in college.
